Does Anthem Cover Rehab?
Anthem can cover rehab when your plan finds the care medically necessary, and many Anthem PPO plans include out-of-network benefits that can apply to treatment here. Bliss Recovery is out-of-network with Anthem, and our admissions team can check your Anthem out-of-network benefits at no cost before you decide anything.
Verify Your Anthem BenefitsWhat Anthem PPO Plans Can Cover for Addiction Treatment
Most Anthem PPO plans include behavioral health benefits for substance use disorder treatment, and those benefits often extend to out-of-network providers at a different cost-share. When care is medically necessary, coverage can reach from withdrawal management through outpatient programming.
Anthem reviews each level of care on its own, and higher levels typically need precertification. On many Anthem PPO plans, the member is responsible for getting precertification when the provider is out of network. We prepare that request with you, so it never sits on your to-do list.
- Medically supervised detox is usually reviewed before a planned admission, with approval granted for a set number of days.
- Residential treatment generally needs precertification before arrival, plus continued-stay reviews while you're here.
- Partial hospitalization (PHP) often needs its own approval, even as a step down from residential.
- Intensive outpatient (IOP) may or may not need precertification, depending on how your Anthem plan is written.
Coverage for Co-Occurring Conditions
Anthem plans generally cover mental health and substance use care under the same behavioral health benefit, so co-occurring mental health conditions can be treated in the same stay when both are documented. We treat addiction alongside bipolar disorder, OCD, borderline personality disorder, and more.

How Anthem Out-of-Network Coverage Works and What You Might Pay
Anthem is a Blue Cross Blue Shield company that does business as Anthem Blue Cross in California and as Anthem Blue Cross Blue Shield in several other states. Its parent company is Elevance Health, though Anthem is the name you'll see on your ID card.
Plan type matters more than the logo. Anthem PPO plans typically include out-of-network benefits, while Anthem HMO and EPO plans generally cover out-of-network care only in emergencies. We aren't in-network with HMOs or with Medi-Cal, including Anthem's Medi-Cal plans.
For out-of-network care, Anthem pays based on what it calls the maximum allowed amount, which can be lower than a provider's charges. Depending on your plan, you may owe an out-of-network deductible, coinsurance, and the difference between that amount and the bill. We'll walk you through each piece once your verification comes back.
Out-of-network claims are usually submitted by the member after treatment, and we supply the itemized superbills and clinical documentation Anthem asks for. Our overview of PPO and out-of-network benefits covers reimbursement in more detail, and private pay and financing options are there if your plan leaves a gap.

What to Have Ready
You bring:
- Your Anthem ID card, front and back
- The subscriber's full name and date of birth
- Your group number and employer name, if your plan comes through work
- The state where your plan was issued, if it's not California
- The level of care you're considering, if you know it
- Current medications and any past treatment programs
We handle the clinical assessment, the precertification request, and the documentation behind it. Coming in from outside Los Angeles? Our page on travel and arrival covers getting here, and our list of what to bring to treatment covers packing.
If Anthem Denies or Delays Approval
A denial isn't always the final word. Anthem's denial notice explains the reason, your deadline, and how to file an internal appeal.
We'll gather clinical records to support the appeal and can request a peer-to-peer review between our medical team and Anthem's reviewer. If the internal appeal doesn't resolve it, many plans allow an independent external review, and urgent situations can be reviewed on an expedited basis.

Why Anthem PPO Members Choose Bliss Recovery
Using out-of-network benefits is a deliberate choice, and it often comes down to wanting a smaller, more private setting. Here, treatment starts with a personalized assessment and moves between levels of care based on clinical need.
- A limit of 12 clients across two residences keeps the setting quiet, with no clinical signage and a discreet admissions process.
- Detox, residential, PHP, and IOP run under one clinical team, so stepping down keeps your care continuous.
- Dual-diagnosis care pairs trauma-informed approaches like EMDR and Brainspotting with individual psychotherapy.
- Admissions handles the Anthem precertification paperwork and reimbursement documentation alongside you.
- Our medical and psychiatric team coordinates with your therapists, so both conditions are treated together.
Bliss Recovery is licensed by the California Department of Health Care Services (DHCS license #191336AP), accredited by The Joint Commission, LegitScript certified, and a member of the National Association of Addiction Treatment Providers (NAATP).

Frequently Asked Questions
Is Bliss Recovery in-network with Anthem?
No. We're out-of-network with Anthem and work with Anthem PPO plans through their out-of-network benefits. A free verification shows what your specific plan allows here.
Can I use my Anthem PPO at an out-of-network rehab?
Often, yes. Many Anthem PPO plans include out-of-network benefits for medically necessary substance use treatment, usually with a separate deductible and a higher cost-share than in-network care.
Does Anthem cover rehab here with an HMO, EPO, or Medi-Cal plan?
Usually not. Anthem HMO and EPO plans generally limit out-of-network coverage to emergencies, and we aren't in-network with HMOs or Medi-Cal. We can still talk through private pay with you.
Will I owe anything out of pocket with Anthem?
Most likely. Out-of-network care typically comes with a deductible and coinsurance, and you may owe the difference between Anthem's maximum allowed amount and the bill. Your verification spells out your plan's terms.
Who submits the claim to Anthem?
For out-of-network care, the member usually submits the claim after treatment. We provide itemized superbills and supporting clinical documentation, and we can answer Anthem's questions along the way.
Can I use an Anthem plan from another state?
Possibly. Anthem plans differ by state and employer, so how yours handles out-of-network care in California depends on the plan itself. Verification confirms it before you travel.
How long will Anthem approve treatment?
Anthem approves care in set increments based on clinical need, then reviews progress before extending. Length of stay is clinically determined, and no plan guarantees a specific number of days.
Is my Anthem benefits check confidential?
Yes. Benefits checks and admissions conversations stay confidential, and we share information with Anthem only as needed to verify benefits and request precertification.
